( 현행과 같음 ) 2. 허가사항 범위를 초과하여 아래와 같은 기준으로 투여 시 Clopidogrel 경구제(복합제 제외)의 요양급여를 인정함. - 아 래 - 가. 투여대상: 가와사키병으로 진단된 소아(만 18세 이하)환자 나. 투여조건 1) 단독요법 : 아스피린에 부작용이 있거나 금기인 경우 2) 2제 요법(Aspirin + Clopidogrel) : 혈전위험이 있으면서 직경 5mm이상에서 8mm미만의 관상동맥류를 동반한 환자로서 warfarin을 투여할 수 없는 경우(와파린에 과민반응, 금기, 국제정상화비율 [INR: International Normalized Ratio] 조절실패 등) 3) 3제 요법(Aspirin + 항응고제※ + Clopidogrel) : 최근 6개월 이내에 관상동맥 혈전 과거력이 있으면서 직경 8mm 이상의 거대동맥류를 동반한 환자 ※ 헤파린, 와파린, New Oral Anti-Coagulant 등 다. 투여용량 1) 24개월 이하 소아환자 : 0.2mg/kg/day 2) 24개월 초과 소아환자 : 1.0mg/kg/day ※ 최대 투여용량 75mg/day 라. 투여기간 - 환자상태 및 관상동맥 확장의 호전 여부에 따라 투여기간을 결정함. (다만, 나목 3)의 경우 Clopidogrel 경구제는 6개월까지 인정) | ○ 교과서, 가이드라인, 임상문헌, 제외국 보험기준, 학회의견 등 참조하여, Clopidogrel 경구제 (복합제 제외)의 허가사용 범위를 초과하여 가와사키병 소아환자를 대상으로 Aspirin에 부작용이 있거나 금기인 경우 또는 관상동맥류 등을 동반한 환자에게 급여 확대함 |
